Deadline extended for University Key Audit

The University Key Audit to survey and verify current keyholders of University-issued hard keys is still underway. 

Anyone who has not participated has until Thursday, March 18 to comply.  Follow these instructions to complete the form:

  1. Before you begin, make sure you have your University ID in case you have not committed your 800 Number to memory. Also, if applicable, have any University-issued keys readily available.

  2. From a desktop or laptop, click the button below to access the form.  The form is not accessible from a mobile device.

  3. ​When the form opens, complete each field with the required information.

  4. If you are attesting that you do not have a University-issued key, after you select “No,” click the “Submit” button.

  5. If you do have University-issued keys, follow the instructions to enter the serial number on your key. There will be a follow-up question in case you are unable to read the numbers on the key. Once you have done this you will click the “Submit” button.
