Conference and Training Request Form

The conference and training request form is a required document that must be completed prior to conferences, training and travel.

Process:

  • Print the form. (Note: Training must be job specific, directly relating to your trade or profession.)
    • ​To print the form, click on either link above. A new window will open with the document. In the top right corner, click the printer icon and click “print.”
  • Complete the entire form and obtain an approval signature from your unit director. (Note: Proof of payment (original receipts, etc.) are required for reimbursement.)
  • In the cost section, make sure to include all information related to registration, hotel, airfare, car rental and food expenses.
  • Submit the completed and signed form with accompanying documentation to FM HR for approval 60 days prior to the event.
